PUMC to Host County Flu Clinic (10/12/24)

Gloucester County is providing a series of free flu shot clinics during the month of October.  One of those clinics will be held in our Fellowship Hall on Saturday October 12.

 

Here are the details:

WHO:  Any Gloucester County Resident (proof of residency will be required)

WHAT:  Vaccination against the current flu strain

WHEN:  Saturday October 12, 2024

TIMEs:  From 10:00 AM to 11:30 AM

WHEREPUMC Fellowship Hall (enter via the parking lot doors)

COST:   Absolutely free!

RESERVATIONS Walk-ons are welcome, but they strongly advise that you complete the online “Consent Form” ahead of time.  See below…

Yep- the clinic will only be open for one hour, but it won’t take long to receive your shot. Now is a great time to take this step to avoid the flu during the upcoming winter season.

Pre-Registration (“Consent Form”)…

Before receiving the vaccine, you must sign a short “Consent Form” (identifying any allergies, medications, etc.)  They “strongly recommend” that you complete this form online before coming to the clinic.  You can fill it out when you arrive, but completing the online consent form beforehand will save you a lot of time when you go to get the shot.

The consent form MUST be completed electronically; they no longer use paper forms.  The Gloucester County Department of Health uses a website called “Metacat Connect” that implements the Consent Form process.

  • The bad news:  If you haven’t created your account before, you must create one in order to complete the online consent form.  
  • The Good News:  Once you’ve created this account, you can use it for any future County sponsored health  services (including flu vaccinations and COVID vaccinations).  

More Good News:  If you don’t want to jump through all of the online hoops, you can simply call the Gloucester County Department of Health office and they’ll walk you through the registration.  Here’s the number:   (856) 218-4102

 

1:  How to Set Up an Account

Before you can access the online consent form, you need to set up a login account with the County Board of Health.  They call this your “Gloucester County Public Health Patient Portal”.  Here’s how to create this account (if you haven’t already done so):

  1. Go to the “New Patient” page: https://gloucester.medicatconnect.com/newpatient.aspx

  2. Fill out the form (the personal information on the bottom of the form is optional). 
  3. Be sure to record your login and password for future use.
  4. Click the “Save” button.

 

2: How to Pre-Register for the Vaccine (Submit Consent form):

Once you’ve created your “Gloucester County Public Health Patient Portal” account, you don’t need to do it again.  You can use this account and login for any future services (including next year’s flu shot!).  If you have the account ready, here’s how to complete the flu shot “Consent Form” (i.e., preregister for the flu clinic):

  1. Log in to your Gloucester County Patient Portal:  https://gloucester.medicatconnect.com/login.aspx   
    (Note that your “Username” is the email address you used when you set up the account.)
  2. Click  on the “Forms” tab (left hand side of screen)
  3. Under the blue bar saying “Consent forms for Specific Vaccine Appointments” you should see a link to the Flu Vaccination Consent Form
    (“Flu Consent English/Spanish”). 
  4. Click on the link for this year’s flu vaccination Consent Form, fill out the form, and click the “Submit” button. 
    (If you filled out the online flu vaccination form last year, this year’s form will be “pre-populated” with the answers you gave last year; if nothing’s changed, all you need to do is to click “Submit.” )

Note that the Consent Form doesn’t apply to any specific clinic location (you don’t have to “reserve” a slot).  Once you submit the form, you can visit any County flu clinic to get the shot.  The form is good for 60 days (after which you need to submit a new form).

 

 

If you don’t want to do all of that, you can just show up at the clinic and they’ll assist you with the consent form.  If you want to pre-register but don’t want to be bothered with the online stuff, you can call the County Health and Human Services office at 856-218-4102 and they’ll assist you.
